New Patient Paperwork
If you are pleased with the initial meeting, our team will further assist you in scheduling an appointment for a formal check-up. Before that appointment, you will be required to fill out new patient forms along with providing any prior dental history/records. We suggest that you arrive at least 20 minutes prior to your scheduled appointment to ensure that you have plenty of time to fill out these forms.
The paperwork is divided into two categories and must be fully completed. One form contains your personal information such as name, age, home address, etc. In addition, you will be required to complete a medical history form. In this form, you must provide complete information regarding past medical treatments, dental problems and, if you have dental insurance, details about the policy and the insurance provider.
When filling out the forms it is important to be precise and provide all the significant details as to avoid any misunderstandings that may or may not have an impact on your future treatments.
For your convenience, our office also provides the choice of filling out the forms before coming to the clinic. This means that you may visit our website and fill out the enrollment forms online here:
This saves you time and ensures an efficient and effective visit. Patients can download the health history forms from our website and fill in all the required details. This information will be stored in our database and evaluated prior to your session. Once our dentist reviews your health history forms and your appointment is set, you will be ready to meet your dentist again and begin your formal dental examination.
After reviewing your dental health history, your dentist will examine your gums, teeth, and mouth. This procedure is known as a comprehensive exam, through which your dentist will observe the state of your dental health and hygiene. An important part of the comprehensive exam is the cleaning. The teeth and mouth are thoroughly cleaned before moving onto the x-rays so that there are no extra debris in your mouth at that time.
Your dentist will look for any sensitivity, decay, and redness around the gums and teeth during the examination, and explain any findings. Even though your dentist will review your medical and dental history in the forms you filled, they may still ask you questions about your teeth while examining them. This is common and can help our dentist identify anything unusual or any information not mentioned in the new patient forms.
After conducting the comprehensive exam, your dentist will take an x-ray of your mouth, which will give a clear picture of the status of your teeth and jaw. These scans are taken to better analyze oral disruptions, diseases, and cavities. The x-rays are suggested to be performed both routinely, once a year, to verify any changes from the last visit and when there is a severe problem.
The panoramic radiographs are the most common types of dental X-rays as they provide the dentist a much more detailed view of the teeth and jaw. The dentist is able to locate any impacted teeth or jaw fractures easily with the panoramic X-ray.
Although believed to be a lengthy process, X-rays usually don’t take more than a few minutes and are completely safe. The procedure is simple and without discomfort. You will be asked to bite down on the gauze and only your mouth will be scanned by the machine. The rest of your body will be protected by a special device. After the procedure, the x-rays will be stored in your file for future reference.
Once the initial dental check-up is completed, your dentist will discuss the results with you. Your dentist will explain any findings such as irregularities or decay in your mouth, along with your overall oral health. They will mention the various treatment options that are offered at our clinic. Your dentist will then suggest the most appropriate and detailed treatment plan for your specific needs, customized to your requirements and your dental issues. You will also have the opportunity to clarify any doubts or questions that you may have regarding your oral health.
After this initial exam, you will receive the total cost of the procedure and discuss the available payment plans, if interested. At this time, you may also inquire about any other costs and the treatments suggested by our dentist. You may also discuss with the doctor any insurance coverage that you may have, in case you are planning to use your insurance to pay for the treatment.
After meeting our dentist at Creative Smiles at WingHaven in O'Fallon, Missouri and discussing possible dental treatments in detail, we will schedule a follow-up session at your convenience. These follow-up sessions will primarily be for cleaning and preparing your teeth for the upcoming procedures.
We suggest visiting our office twice a year to prevent any cavities, tooth decay or any other oral health issues. During these routine checkups, the teeth are usually cleaned and treated to ensure that they remain in good shape. In addition, during these routine check-ups, your dentist can track your oral health condition and work on any preventive procedure to maintain your teeth in good shape.
